Are you an Indiana Medicaid member resident who has no means of transportation to get to and from your medical related destinations? WellTrans is here for you! WellTrans arranges transportation services to Indiana Medicaid members who require non-emergency transportation assistance to Medicaid-billable healthcare services. Call WellTrans to arrange your non-emergency medical transportation service at least two (2) days in advance of the appointment date. You can schedule up to 45 days in advance.

Making Transportation Arrangements:

When scheduling transportation services, please have the following information ready for the Customer Service Representative:

  • Member’s full name, phone number, address, date of birth and county of residence
  • Member’s Medicaid number as it appears on their Indiana Medicaid card
  • Specify any special needs (e.g., oxygen, escort, etc.)
  • An emergency contact’s name and phone number
  • Destination information – address, phone number, doctor’s name, type of appointment, and name of the medical facility to which you are being transported.
  • Member’s Mobility status (e.g., waking, wheelchair, etc.)
